Print out this form on both sides, fold, affix a stamp and send, OR, print on two pages, put the pages back to back, fold them and tape them at the top, affix a stamp and send.

Absentee ballot applications are sent to the County Clerk. They must be recieved by May 27. After that voters may apply for an absentee ballot in person at the County Clerk's office up to June 2 at 3pm.

Unaffiliated voters may vote by absentee ballot. They will be sent both ballots and can send back the one for the party of their choice.
